RN Field Case Manager – Hospice (North Miami-Dade County)

This position isbased in Hialeah, FLand requiresdaily travel throughout North Miami-Dade County. Schedule:Monday-Friday, 8:00 AM - 5:00 PMwith participation in an on-call rotation.

Requirement:

Must be fully bilingual in English and Spanish,with the ability to communicate effectively with patients, families, and team members in both languages.

Salary Range:

$80,000–$85,000 annually, plus bonus opportunities.Final compensation may vary based on experience and skills.

Orientation, Onboarding & Training

You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.

Responsibilities
  • Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
  • Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
  • Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
  • Records pain/symptom management changes/outcomes as appropriate.
  • Teaches patients, families, and caregivers appropriate patient care techniques.
  • Regularly communicates with clinical manager and care team.
  • Performs on-call responsibilities and on-call services to patients/families as assigned.
  • Other duties as assigned.
Qualifications

Required:

  • Full bilingual proficiency in English and Spanish
  • Current RN licensein the state(s) where you will work
  • Minimum 1 year of strong clinical RN experiencein an acute healthcare setting
  • Current CPR certification

Preferred:

  • Hospice or home health experience
  • Strong communication and interpersonal skills
  • Ability to work independently and manage multiple priorities
  • Comfortable with daily travel between facilities
  • Proficient in electronic medical record (EMR) systems
